Hospital at Home gives members the choice of treatment in the comfort of their home for various illnesses. This way, they can be with their loved ones while they are closely monitored and treated with world-class medical resources.
Discovery Hospital at Home is powered by integrated, cutting-edge technology and supported by highly skilled clinical staff, clinical protocols and best clinical practices as well as appropriate medical scheme benefits. Discovery Health Medical Scheme (DHMS) members admitted to Hospital at Home have access to enhanced benefits and services delivered through their personalised care team. Together, these benefits and services ensure a seamless healthcare experience for patients, making them healthier and enhancing and protecting their lives.
Home-based Hospital Network
DHMS members on the Delta, Smart and KeyCare plans will need to use the Home-based Hospital Network for select low-acuity conditions.
The Network consists of Discovery Home Health, Mediclinic and Quro Medical. All services offered within the Network are funded from a member's Hospital Benefit where there is a valid preauthorisation in lieu of hospitalisation.
Remote monitoring and clinical insight
24/7 real-time remote monitoring supported by cutting-edge digital healthcare technology
A patient's condition is monitored 24/7 through cutting-edge remote monitoring technologies. These send data to a secure dashboard that is monitored by the patient's Home Care nurse, Discovery care co-ordinator and team of command centre doctors. You can also view your patient's vitals on-demand, on the secure dashboard.
Hospital level care and treatment
Hospital-level diagnostics and intervention in the comfort of home
Patients get access to a range of clinical diagnostic procedures and interventions to manage medical or postsurgical conditions with hospital-level care in their home. We support this with extra benefits for which we pay from risk (not the patient's day-to-day benefits) to improve the member's experience.
