What is Discovery Home Health?

Discovery Home Health brings hospital-level care to the comfort of patients' homes. Through Discovery Home Health, stable patients who would otherwise be treated in a general ward can access at-home, hospital-level care, on either a 24-7 (short-term) basis or on an episodic basis, for a wide range of conditions.

24/7 clinical oversight from a care team

Physical and virtual 24-hour care is delivered by a dedicated team that includes doctors, nurses and allied healthcare professionals.

 
Remote monitoring

24/7 real-time remote monitoring supported by cutting-edge digital healthcare technology

A patient's condition is monitored 24/7 through cutting-edge remote monitoring technologies. These send data to a secure dashboard that is monitored by the patient's Discovery Home Health nurse, Discovery care co-ordinator and team of clinicians in the ER Consulting clinical command centre. You can also view your patient's vitals on-demand, on the secure dashboard.

 
Hospital-level care

Hospital-level diagnostics and interventions

Patients get access to an enhanced range of clinical diagnostic procedures and interventions to manage medical or post-surgical hospital-level care in the home, supported by additional risk-funded benefits to enhance the member's experience.

Low-acuity conditions where repeat, episodic care is required

Quality home-based care services for patients who require postnatal care, IV infusions, advanced wound care, and palliative care.
